If you agree that your minor child may participate in Quest Hunt Co.’s competition pursuant to the Official Rules, Participation Agreement, and the terms of this Consent Form, you may electronically agree by checking the consent box during the registration process.

By checking the consent box, you, as the parent or legal guardian of the participant, acknowledge and agree to the following:

• I hereby give my consent for my minor child to participate in the Quest Hunt Co. hunting competition conducted by Quest Hunt Co.
• I irrevocably grant my permission for Quest Hunt Co. to use my minor child’s likeness, name, and any photos submitted for purposes related to the competition, including advertising, marketing, publicity, and promotion, in any media, now known or hereafter created, in perpetuity.
• I confirm that my minor child and I reside in the United States of America.
• I certify that my minor child has obtained all necessary licenses and/or permits to legally harvest a deer in the state where the competition will take place.
• I confirm that my minor child and I have read and understand the hunting rules and regulations of the state where the competition will take place.
• I agree that my minor child may be required to submit to a lie detector test if their team finishes in the top 10 of the competition.
• I agree that my minor child is eligible to accept any prizes awarded as part of the competition.
• I understand that my minor child’s name and other relevant information may be posted publicly on Quest Hunt Co.’s website or other platforms in connection with the competition, without further notice or permission.
• I confirm that I am a legally competent adult, and I have the authority to contract for the minor child named in this registration.
• I have read and understand the Official Competition Rules and this Parental Consent Form, and I voluntarily agree to the terms set forth herein.
• I consent to provide necessary information regarding my minor child to enter the competition and to receive any awarded prizes.

By checking the consent box during registration, I acknowledge that I am bound by the terms of this form, the Official Competition Rules, and the Participation Agreement, and that this consent applies to me and my minor child, and is binding upon our heirs, legal representatives, and assigns.
